ABOUT US
The Sherwood Park – Strathcona County Primary Care Network is a group of health care professionals and administrative staff who work with you and your Doctor to help you live well. Our caring team provides education and support for managing chronic diseases and mental health concerns, while providing access to specialists and services all within your local community.
We are currently looking for a Registered Nurse, to join our team in Sherwood Park and Fort Saskatchewan offices.
- Permanent 0.6 FTE (23.25 hours/week)
- 3 shifts/week; Tuesday to Thursday 8:00 PM – 4:15 PM, may require one evening shift per week (12:00 PM – 8:15 PM)

POSITION SUMMARY
The role of the Sherwood Park – Strathcona County PCN Family Practice Nurse – RN is to provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The RN will assess, develop, implement, and evaluate care plans for patients. The RN will work to full scope of practice with an emphasis on providing comprehensive care, education and guidance to patients with chronic disease.
KEY RESPONSIBILITIES
Include, but are not limited to the following:
General
- Assessment of patient conditions, needs and goals in collaboration with the patient, attending physician and multidisciplinary team
- Identify needs and set priorities in collaboration with the patient and other team members
- Provide short term interventions as needed
- Facilitate appropriate referrals to community resources and programs
- Provide relevant patient education to prevent and manage clinical conditions
- Participate in evaluation of the clinic and its services
- Assist in planning, developing, and monitoring for services provided
- Assist in the development of policies and protocols for chronic disease management, geriatric care, maternal/child and mental health
- Develop systems and communications protocols to facilitate patient follow up
- Assist in setting up health promotion programs relevant to the needs of the practice population.
- Assist in developing and implementing patient self-management programs.
- Assist in the implementation of strategies for improving the quality and provision of care for the practice population
- Other duties as required.
EDUCATION & EXPERIENCE
- Baccalaureate degree in Nursing preferred
- Five years or more of related nursing experience
- Current CRNA Registration, CPR, and BCLS
- Initiative, accountability, decision-making, interpersonal, and communication skills
- Demonstrated ability and willingness to work in a dynamic environment that requires flexibility and balancing of competing priorities
- Experience in PAP testing
- Experience working in a team setting with other health professionals
- Advanced skills in caring for patients with chronic, complex, and/or specialized needs
- Certified Diabetic Educator (CDE) would be considered an asset
- Experience in establishing protocols for program startups, especially developing plans and priorities when workload varies
SKILLS & ABILITIES
- Expert knowledge of nursing processes and practices including intervention strategies and therapy
- Strong, effective written and oral communication skills, high level of perceptiveness

- Ability to build a good working relationship with healthcare professionals, clients and other members of the community

- A strong knowledge base in medications and is able to conduct medication reviews with patients
