Full job description
The Pathway Manager provides operational oversight and leadership to the wider Neurodevelopment Support Service Team by ensuring: The Navigation Team complete comprehensive and holistic assessments of need for patients with suspected ADHD who fit the criteria for the service within agreed timeframes. Risks are identified, assessed for urgency and appropriately escalated to the appropriate person or service when required. The Navigation Team work from the agreed work premises and contribute to the overall smooth running of the services. The Navigation Team develop collaborative wellbeing support plans with individuals with ADHD symptoms which promote choice and complement any medical treatment or advice.
The Navigation Team discuss alternative providers of ADHD assessments enabling clinically appropriate and informed patient-choice as part of a joint management plan. In cases where it is decided to refer, all necessary clinical admin will be completed as per agreed protocols. The Navigation Team identify any barriers to people accessing support for their ADHD symptoms and work with individuals to overcome these. The Navigation Team use a range of techniques and interventions to support people with ADHD symptoms, to better understand themselves from a strengths and challenges perspective, to encourage a more positive mindset and ability for self-advocacy.
The Navigation Team monitor and record outcomes within SystmOne or EMISweb using agreed templates and assessment tools and outcome measures. Build networks with local community resources such as activities and services that may have an impact on the health and wellbeing of people living with ADHD symptoms and support individuals to access these. Identify gaps in provision for people with ADHD symptoms within the current Health and Care Partnership to inform future service requirements. Develop multi-agency working across communities and voluntary and statutory organisations to improve health and address health inequality for people with ADHD symptoms.
Gather and collate statistical and other information and data as required, to report on activity and outcomes and ensure effective qualitative and quantitative monitoring and evaluation of the service. Reflect on practice, and participate in team meetings, practice development forums and peer supervision within the service. Provide supervision to the Neurodiversity Care Coordinator and Service Support Officer as required. Ensure that client records and other information systems are completed accurately and within agreed timescales.
Ensure that support undertaken is meaningful and respectful, promoting self-understanding and self-compassion, and is in line with the individual support plan. Work in partnership with the Clinical Lead. Adhere to organisational policies and procedures relating to risk and personal safety. Support and mentor volunteers where appropriate.
Work with team members and colleagues to continually improve the quality of services within the overall organisational governance frameworks and service objectives. Any other duties commensurate with the grade and level of responsibility of this post.
